A CBC Story – How the Q&A helped change my Life?
One question: Does anyone else have low WBCs/neutrophils?
Does anyone else experience repeated below-range CBC white cell and neutrophil counts?
My WBCs are always just at or below the lower end of the range of 4.2 – 9.1. My readings for the last four draws were 3.6, 4, 4.5, 3.9, and 4.9. The two most recent readings were drawn in the last two months, and the three prior readings were from the prior 3-4 years.
Neutrophil counts usually in range at around 2.3 (range 1.8 to 5.4) but most recently was 1.6
From what I read, the reasons for this range from terrifying to nothing concerning at all, and I’m wondering where I land in that range. My primary care physician signed off on the bloodwork without an issue but then referred me to a haematologist only after I pointed out to him that I’d been below the WBC range in 3 out of the last 4 tests.
I feel like I am not often sick.
Like any average person, I get 1-2 colds yearly. I am not lightheaded much or have any significant health issues other than cholesterol, which I just started medicating last week.
Has anyone else experienced this?
Answer:
Physician here. A chronically low white count in an otherwise young, healthy person is a normal finding. It’s called benign neutropenia and is more common in folks of African, Middle Eastern, Mediterranean, and West Indian descent, though it can still be found across all population groups. It’s a fun fact actually, but our reference range was WBC and is standardised on traditional white males. So it doesn’t consider the heterogeneity across most populations. I wouldn’t be concerned with that laboratory result value.

Low to Normal WBC
- For some people naturally have counts on the lower side. And for many, this is perfectly normal and harmless, particularly if it doesn’t drop drastically or cause symptoms like frequent infections. But minor CBC fluctuations. For example those between 3.6 and 4.9, can happen due to various factors like stress, mild viral infections or even dehydration. Regular monitoring by your healthcare provider, which you’re already doing, is often enough.

Neutrophil Levels
- Since your test counts generally stay within the normal range. But this is reassuring, as they are indeed crucial components of an immune system. And the slight dip to 1.6+ may actually be a transient change. And potentially related to some minor infections, stress, or natural variation.

Referral for Haematology exams
- Maybe referral to a haematologist is likely the best precaution option, especially since this pattern has been ongoing for awhile. Also Hematologists can rule out any rare causes, such as chronic neutropenia. That is usually harmless but can cause lower-than-average counts, especially if it’s persistent. They may run additional tests, like checking for immune markers or vitamins (B12, folate), which can affect blood cell production.

Good General Health
- The fact that you don’t get sick often and experience minimal symptoms is a very positive sign. Many people with mild, consistent neutropenia live without any significant health issues as long as they don’t have any symptoms like frequent infections, fatigue, or prolonged illness.
I would get that high IPF worked up further, however. It could be a one-time error or a side effect of certain medications. It just needs to be investigated further.
Question: MVP is high; should I be worried?
I have a CBC w/Diff done (For Checking a Hypothyroidism, which of course it turns out I don’t have). And everything was fine on my Metabolic Panel (Liver / Kidney tests / etc… But besides 1 thing was off and being a hypochondriac it has me very worried).

Concerns About MPV
So, as you can see, Everything looks good besides MPV. I’ve read a high number can be a few things like Cancer, chronic myelogenous leukaemia (CML) (or other leukaemias), ITP or something called heterozygous thalassemia. So, I usually don’t worry, but my platelets tend to be on the lower end (they always have been, though they remain above the normal range). My past 3 results have been 186 149 184. Which are low but still normal.
This has been over 3 years
My MPV has also been 12 back in a test I took in 2016 (The doctor has a site that shows your past results conveniently). Anyways…do I need to be worried? I know it’s only slightly over. I was lacking sleep horribly. and had some alcohol the night before (Not sure if that would affect it) and had done some exercise the day before (Which not sure if that would affect it either). Sidenote….is my platelet count worrisome? Since it’s always towards the low end? (It’s always been that way).
Thankfully, the rest of my results in the past have always been perfect. And the same with my Comprehensive Metabolic Panel.
Do you know if I need to get this retested?
How likely is this slight overage to be some of the diseases listed above? (It seems unlikely since everything else is normal.) Thanks. I know this is worrying too much, but understanding why I don’t need to worry would be helpful!
Answer:
I’m an MD but not a haematologist, so if someone steps up to correct me, I will trust them instead. But MPV is one of the CBC values that doesn’t mean much.
When interpreting a CBC, I look primarily at certain values: cell counts (RBC, WBC, platelets), haemoglobin, and hematocrit. If there’s a differential, I check to see if it’s skewed towards any predominance.
Understanding Abnormalities
Most other values (hint: most of the ones with “mean” in the name) help characterise any abnormalities in the main levels. So, if Hemoglobin is low, I want to check MCV to see if I have macrocytic or microcytic anaemia. If it is just outside the normal range and everything else is normal, I don’t really care. Normal ranges aim to encompass most, but not all, normal variations within the population.
For this specifically, platelets are larger when they are young. So when the test happened, you had a slight preponderance of young platelets compared to old. Platelets only last 8 to 9 days. Your platelets may die on the earlier side of that range, so your mean platelet volume will always skew a bit large. In the absence of abnormal platelets (which yours are not) or specific symptoms, this is not worth worrying about.
